TRAINING RISKS: I understand the risks involved when participating in Elite Kung Fu's athletic activity. I agree that the participant(s), whether myself, child, or minor, are medically and physically able to be involved in such athletic activity. I understand that I should consult a physician before any type of athletic activity. I discharge, waive, and release any claims for losses or damages, mental and/or physical injury, or death, that may take place during or after participation with Elite Kung Fu. I agree to assume these risks. I agree to indemnify and hold harmless Elite Kung Fu or any director, officer, employee, volunteer, or coach associated with Elite Kung Fu from any losses, damages, costs, injuries, or death that may occur with Elite Kung Fu activity in any facility or location (outdoor or indoor) in which Elite Kung Fu operates. I agree that I am responsible and liable for such costs, losses, liabilities, damages, injuries, deaths that may occur and I promise not to sue Elite Kung Fu or any director, officer, employee, volunteer, coach, or person associated with Elite Kung Fu.
REFUND AND ATTENDANCE POLICY: There will be no refunds for the program. It is the responsibility of the student to show up to his/her scheduled program.
MAKE UP POLICY: Please email Elite Kung Fu to schedule the make-up class.
PHOTO RELEASE WAIVER: I hereby grant Elite Kung Fu the permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of Elite Kung Fu and will not be returned. I hereby irrevocably authorize Elite Kung Fu to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the Elite Kung Fu from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
Code of Conduct
- All participants at Elite Kung Fu are expected to maintain high standards of moral and ethical conduct that includes self-control and responsible behavior. Both athletes and parents must display courtesy and good manners and consider the physical and emotional well being of others by treating everyone with respect.
- No bad behavior, disrespect of others and/or their property, or abuse of the equipment and/or the facility will be tolerated. Be respectful to yourself, others, and the facility.
- Criticizing other Students is not tolerated. Support fellow to do their best. Show good sportsmanship towards your teammates and peers. Celebrate everyone’s success.
- Encourage yourself and others to build bonds with fellow teammates and competitors.
- Work together as a team
- Strive to develop a strong work ethic by increasing self-image with training in an environment containing both discipline and trust.
- Children misbehaving, creating a distraction, or being a risk to other students will be asked to sit out. Three times in a session and their parents will be contacted, if misbehavior continues, you will be asked to withdraw your child from their class.
